1. Michigan Automated Prescription System (MAPS) Requirement
Michigan Board of Pharmacy Administrative Rule 338.3162b states all pharmacies, dispensing practitioners and veterinarians who dispense controlled substances in Schedules 2-5 are required to electronically report this prescription data to the Michigan Automated Prescription System (MAPS).

Your pet's treatment plan includes a prescription medication for a controlled substance.

In order for AEC to dispense a controlled substance we are required to verify the identity of the recipient using a valid driver's license, social security number or state ID card, and to report this information to the State of Michigan.

2. Controlled Substance Recipient Identification
Your personal information is protected. The birth date and ID number you submit will be securely encrypted. Only the last 4 digits of the ID you enter will be visible to AEC employees. Your ID information will be securely transmitted to the State of Michigan and subsequently removed from AEC systems following data confirmation (normally within 24-48 hours).
The recipient identification information entered on this form must be completely accurate. I understand that if this information is rejected by MAPS for any reason that I will be contacted by AEC and asked to validate this information either verbally or in person.

I understand that AEC deletes my identification information following transmission to the State of Michigan Automated Prescription System and confirmation of data acceptance.
